The office is located at the corners of Washington Street and John Street, in front of the Chemung County Health Department/Nursing Facility. We are in the Peter Chalk Pavilion. (see Walk-in service )

Birth and deaths that occurred in Chemung County ONLY are available in this office. 

We cannot order or process or hold certificates from another city, county, state or country.  You must contact the county, city, village or town clerk in the State where the birth/death occurred, or go directly through the Department of Health – Vital Statistic Office in the State where the birth/death occurred.

Birth and death certificates may only be requested by an authorized person. (See Who Is Eligible To Request A Birth Certificate  or  Who Is Eligible To Request A Death Certificate)  Requestor must have proper ID and proof of relation OR legal need in some cases.  (See Acceptable ID and Documents ).

Birth and death certificates and the indexes relating to births and deaths are not subject to the FREEDOM OF INFORMATION LAW (FOIL) and are not open to public inspection.
