Home Health Care

Chemung County Health Department Home Health has been serving Chemung County for over 40 years. We are a nonprofit agency that serves people of all ages. We specialize in pediatrics, cardiac, and respiratory disease management and rehabilitation. CCHD accepts most insurances, and we will work with you to get the care you need in the comfort of your home.  

Our Mission

Chemung County Home Health Care promotes the health and well-being of our community by empowering our patients to achieve their health care goals.  Providing clinically excellent compassionate care and patient centered service is our pormise to our patients.


Our professional staff of physical, occupational, speech and respiratory therapists are here to assist you with your rehabilitation needs.  We provide services to those who have suffered a functional loss due to injury or a disabling illness.  We design an individualized care plan with your needs and goals in mind and we do this in the comfort of your own home.

Home Health Care Services

We provide professional home health care, specializing in actue and chronic disease treatment to help you and your family manage your illness at home.  The following professional health care services are available as prescribed by your physician:

*Case management (care coordination by our nursing staff).
*Infusion therapy.
*Wound care management.
*Diabetes counseling.
*Education to promote independent living.
*Medication management.

*Licensed physical therapy, speech therapy, occupational therapy, medical social work, nutritional services and home health aide visitation provided in conjunction with skilled nursing and case management. 


To be eligible for homecare, there are certain requirements that need to be met.  Home health care is covered by many insurances including Medicare and Medicaid.  Some insurances, such as Medicare and some private insurances require you to be homebound.  For more information on requirements for home health care, please call 607-737-2028, press 1.
